For your convenience, we have answered a variety of frequently asked questions.
Scheduled appointments are necessary in order to see a provider. We will make every effort to schedule an appointment the same day as your request. Your primary physician will be updated on all services provided by our office.
PLEASE ALLOW 1 1/2 TO 2 HOURS FOR YOUR INITIAL APPOINTMENT. Your appointment will include a consultation with a physical exam and specific allergy and asthma testing if needed.
Please bring ALL of your medications (not just allergy medication), your insurance card and referral if one is required. Previous medical records will be requested at your first visit.
Many insurance plans require a referral for office appointments and shot visits. It is your responsibility to obtain the appropriate referral from your primary care provider PRIOR to your visit. Any fees for unapproved visits will be your responsibility.
Insurance policies are contract's between you and the insuring company. As a courtesy, one of our staff members will file your insurance claim, provided we have your current assignment of benefits. You will be responsible to pay your copay, coinsurance and/or deductible at the time services are received. If your insurance company does not pay us within a 90-day period, you will be held responsible for payment.
We are contract with most insurance providers. To verify if we accept your insurance, please call or email our Insurance Referral Specialist.

At your first visit, one of our staff members will call your insurance company and obtain a list of benefits that your insurance covers. These benefits will be recorded and should remain the same as long as you keep the same insurance plan and remain current with your insurance payments. However, the benefits given are not always a guarantee, and we will not be responsible for incorrect information.
We welcome all questions you may have regarding your fees, account balance, or insurance payments. During business hours, an Account Representative or our Practice Administrator will be available to assist you with any concerns or questions you may have.
For your convenience, we accept Visa, Master card, checks or cash.
Skin testing is performed in order to help identify what aeroallergens you may be allergic too. Testing will consist of a scratch and/or intradermal (just below the skin) tests. Generally, skin testing is well-tolerated, even in very young children. Skin testing takes 20-25 minutes, recommendations and options for treatment are discussed during your visit.
If you would like to be skin tested on your initial visit, all ANTIHISTAMINE Medications must be stopped 5 days prior to your scheduled appointment. YOU SHOULD COUNTINUE ALL OTHER MEDICATIONS (Please click here for a common list of antihistamines).
Immunotherapy Injections are given on a walk-in basis. We require that you wait in our clinic for 20 minutes after receiving an allergy injection. Please arrive no later than 20 minutes before the clinic closes to receive an allergy injection/immunization.
Please call in your prescription refills 48 hours in advance. Please have the phone number of your preferred pharmacy, this will further assist our staff and speed up the prescription refill process. For your safety, refills will not be called in if you have not been evaluated in our office within the past year.
To reschedule an appointment, we please ask for a 24 hour notice. Canceling your appointment the same day as the appointment is a "no-show". Three no-shows within a six-month period can result in a discharge from our practice. We reserve the right to charge a $10.00 fee for no-shows and cancellations without 24-hour notice. This fee is not covered by your insurance carrier.
Please leave a message with the answering service. An on-call staff member will be reached for URGENT concerns only. Prescription refills will be handled the next business morning.
When making a request for FMLA, disability forms, etc, please allow five business days for our medical personnel to complete these forms. There is a $25.00 fee for completion of each form.
